I need some help with this one. The double impression of the neck caught my attention. And the double clash of the back shoulder on the reverse got me thinking it is some sort of MAD clash (like the 1983 with the extra letters). What do you think?

It is definitely a double clash. As far as a MAD clash, technically, it could be described as one. The 'extra letters' clash and this one is really not on the same level.

The extra letters on the two 1983 Lincoln cents and the Delaware quarter "Rodney Caesar" are what is called a counterclash and are formed by a foreign piece of metal that is struck by the die and then in turn striking the coin. This occurrence is high unusual due to the circumstances involved in producing this error.
