Nice clogged die errors... Here is a 42 I came upon.

It's a Grease filled Die. It happens when the operator starts production of the run. Grease is applied to the de. When the die strikes the planchlet with force, the grease has no where to go. Hence: The coin is struck without the letter or digit struck without the impression.
